All children must have all forms completed before they can be dropped off at Open House without a parent.

You can complete most of the forms online via our portal.

In addition to the three online forms, which include a medical form, your child’s doctor will have to complete and sign the Child Health Examination form (CHE). This form is required by the Department of Health and must be renewed every year on or before the date of exam on the form (in the middle on the left side), not the signature date on the bottom. You may need to make an appointment with your child’s physician to get the form completed. The completed form can be uploaded directly into the portal.
